Sir,

I have the honor of reporting myself at New York, having been sent hither on account of a wound received at the battle of Gaines' Mill and ill health by the order of Surgeon Letterman Medical Director of the Army of the Potomac.

I respectfully request that the enclosed applicaton for leave of absence may be forwarded without delay and that the reply be sent to me at Matteawan, Duchess County, New York.
