Document Signed, 4to., Camp Pierpont, March 2nd, 1862, "Invoice of Ordnance and Ordnance Stores, turned over by Thomas Chamberlin Cap. "D" Co. 5th Reg. Pa. Res." and signed by him. 

Chamberlin also served in the 150th Pennsylvania Infantry and was wounded during the unit's heavy engagement in the Battle of Gettysburg, Pa. on 7/1/1863. 

An account has him sitting on a table in the McPherson tenant house and opening his shirt, exposing to the writer a ghastly wound through the breast; the bullet coming out his back.
